Home Health RN / Registered Nurse New

HealthCare Support is actively seeking a Home Health RN / Registered Nurse to fill an opening with a prestigious organization in South Carolina. Location – Columbia

Daily Responsibilities for Home Health RN / Registered Nurse:

• Use the nursing process (assessment, planning, implementation, evaluation) to meet patient care goals
• Assess physical, psychosocial, and environmental factors to develop individualized care plans
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Educate patients and caregivers on disease management and home care practices
• Document patient progress, teaching effectiveness, and ongoing care needs
• Schedule: Monday – Friday, 8:00am – 5:00pm
• Minimal On-Call
  
  
  

Required Qualifications for Home Health RN / Registered Nurse:

• Active RN License in SC
• At least 1 year of RN experience
• Active Drivers License
